Usage of Information
Inquiries are emailed to the Great Lakes Medical Transit business inbox to evaluate service needs and respond. The website uses Replit hosting, Resend email delivery, and the receiving email provider to process the submission. Do not include confidential patient information. Business inquiries are not submitted to a public directory or mailing list by this form.
Technical Information and Retention
The hosting and email services may process technical information such as IP addresses and delivery logs. The inquiry endpoint uses a temporary IP-based limit to reduce spam. Inquiry content is sent to our email inbox rather than saved as an order in the dispatch system. To ask about an inquiry or request its deletion, contact the address below. The portal uses a session cookie when you sign in.

Our public contact forms are designed strictly for business-to-business logistics inquiries. Users are explicitly instructed to never submit patient names, prescription details, or any other Protected Health Information (PHI) through the public marketing site.
